Peer Team Leader - Regional Peer Network Coordinator (RPNC)

Independent Living, Inc., a progressive and dynamic cross-disability services organization and a leader in the provision of peer services in the Hudson Valley region is seeking to hire a Peer Team Leader / Regional Peer Network Coordinator (RPNC) in Monticello.
As a Peer Team Leader / Regional Peer Network Coordinator (RPNC), you will work provide daily direction and oversight of the peer staff at the Monticello office, attend meeting throughout Sullivan County, work within partnerships such as Mobile Mental Health as well as provide mental health support and advocacy efforts. Must have personal “lived” experience as a former or current user of mental health services and be comfortable with self-disclosure as needed. Must possess a valid driver’s license and have reliable transportation. Bachelor’s degree in human services plus two years of advocacy experience, preferred.

DUTIES AND RESPONSIBILITIES:
- Provide daily direction and oversight of the Peer staff at the Monticello location in conjunction with the Site Manager including, but not limited to:
- Oversee program goals, compliance and initiatives in conjunction with the Site Manager.
- Staff scheduling, monitoring time and attendance and conduct performance.
- Establish and maintain effective working relationships with participating 9.39 hospitals, other treatment facilities, correctional facilities and community providers.
- Document all interventions with the people receiving services in the person’s individual Electronic Health Record (EHR) and submit reports as required.
- Complete all reporting and record keeping as required by the agency and other regulatory entities.
- Participate in in-service, informational sessions and trainings as applicable.
- Attend relevant Sullivan County Co-occurring meetings including but not limited to:
- Inpatient/Outpatient Mental Health Providers’ Meetings
- Single Point of Access (SPOA) Meetings
- Sullivan County Jail Meeting
- Work with people throughout Sullivan County Work in partnership with the Mobile Mental Health to reduce hospitalization and recidivism.
- Participate in discharge planning.
- Engage individuals in the goal setting process to improve their health and well- being and follow through with their discharge plan.
- Facilitate and support the development of an integrated support network for individuals which includes, but is not limited to, case management, benefits advisement, supported housing, self-help, employment and educational services, in addition to other community supports individuals may wish to access.
- Provide mental health peer support, coaching, education and assistance related to:
- Attending medical appointments with their primary care providers
- Filling prescribed medications
- Medication self-management
- Health and wellness
- Engage in individual and systems advocacy.
- Educate individuals, families, and community resources about challenges, options, and resources with regard to persons with disabilities.
- Promote recognition of the value of holistic peer support as an essential component of a comprehensive approach to prevention, treatment and continuing recovery that must be interwoven into both our regional and state mental health/substance use service systems.
- Cultivate current and newly formed relationships with regional grass roots organizations, collaboratives and groups created to advocate for peers in recovery, organized around issues of intersectionality and the identification of wellness and mental health disparities, and those advocating for peers involved with the criminal justice system.
- Network with mental health providers and community support services to maintain an integrated support system for persons with psychiatric disabilities as well as those in Co-occurring programs.
- Represent the WCNY-IPA RPN regionally supporting and promoting regional peer services and building region wide peer service capacity.
- Participate on local planning councils, advisory committees, boards and coalitions of consumer-controlled organizations, and in other neighborhood, regional and community-level activities, both for the purpose of representing the RPN and promoting peer services at all levels of our region’s mental health infrastructure.
- Serve as a catalyst, capacity builder and broker to harness the power of social media platforms and virtual technologies in disseminating information pertinent to peers and peer organizations located throughout our region. This will include press releases, public service announcements, and flyers.
- Support the RPN by implementing strategies aimed at sharing and presenting information about the IPA, promoting IPA membership and collaboration efforts, supporting and contributing to the capacities of IPA members, building collaborations and develop initiatives to promote peer services, providing technical assistance to service providers, collecting and share peer services data, and identifying peer service gaps and implementing plans to build capacity.

SHIFT:
- Monday through Friday 9am-5pm.
PAY RATE:
- $23.45/hr - $24.43/hr

QUALIFICATIONS:
- Bachelor’s degree in human services plus two years of advocacy experience preferred.
- Must complete approved advocacy training programs within the first six months of employment.
- Must be a “peer” and have a working knowledge of the mental health system and recovery process (including but not limited to 12 step programs, Self Help and other alternative methods).
- Must have personal “lived” experience as a former or current user of mental health services and be comfortable with self-disclosure as needed.
- Bilingual English/Spanish strongly preferred.
- Honesty, respect for diversity, high personal standards, language and behaviors reflecting such values and the Independent Living philosophy.
- Demonstrated ability to recognize the need for and facilitate connections between consumers and IL and other disability related services.
- Knowledge of local, statewide and national disability related issues and community dynamics.
- Maintain a current and working knowledge of systems issues.
- Working knowledge of confidentiality requirements related to records pertaining to Mental Health and Chemical Dependency laws.
- Excellent written and verbal presentation skills.
- Must have good time management skills and be self-directing, especially if there is any downtime.
- Must possess a valid driver’s license and have reliable transportation.
